- Get your vet to complete the veterinary referral form (Downloadable) ensuring it is signed and dated. This must be posted, faxed or brought to us in person before your dog can commence any treatment.
Many vets have our referral form on file in which case a quick phone call to your vet is all that is needed. All you need to do is ask them to fill in a referral form for your dog to receive hydrotherapy at Cherrytree and they will take care of the rest.
